Members of the Unified Medical Staff and Unified Allied Health Professional staff have access to their patients’ hospital based medical records through membership. Members may additionally obtain read-only access to the medical record for their office staff or agents for continuity of care and billing purposes. Each user will have a unique user ID and password and may not share it. The form below must be completed in its entirety and submitted as instructed to obtain such access.
